Parents agree that all full year fees are paid on a monthly basis and will be drafted on the first of each month in advance. Daily pass fees are payable per occurrence. Full year fees are based on booked days, not attendance, therefore parents are responsible for fees whether child rides or not. (This includes club days, bus suspension, sick days and family vacation time). NO REFUNDS WILL BE GIVEN FOR EARLY WITHDRAWAL. Rate will be pro-rated according to start date. Any parent who gives a full one month notice of temporary withdrawal such as family vacation will have their fees reduced by 50% for the period of time of withdrawal. No regularly scheduled services shall be rendered without payment in full. Payments for all monthly scheduled routes will be made by automatic reoccurring draft from credit or debit cards, or bank accounts. Fees are non-refundable. All rates are based on destinations located within 0-45 miles from pick-up point. There may be a 7% surcharge added to the standard fees when gasoline prices rise above $4.00 per gallon. Any damage to the bus that is willfully caused by your child will be replaced or repaired at the cost of the parent. If parent is not at the stop at when the bus arrives, the child will ride the bus to the Academy of Martial Arts at Stop A and parents will be contacted. There will be a late fee assessed at the time of pickup. Termination Eagle Bus Service reserves the right to suspend or terminate services for any child without notice, should it be deemed necessary for the overall safety and well-being of other children on the bus. Withdrawal Parents agree that a minimum notice of one full month (notice given on the first of any given month) will be given for permanent withdrawal of any child from service or agree to pay a months fee in lieu of appropriate notice. SAFETY IS OUR GOAL We know you share our concern for the safety of every student who rides a school bus or activity bus. We ask you to impress upon your child that it is absolutely necessary to follow the rules and to obey the bus driver. It is very important that you review these rules and responsibilities with your child. Students must obey the directions given by the bus driver or monitor. Students must be seated while the bus is in motion or stationary unless embarking or disembarking. Quiet polite talking will be used. There must be care and cleanliness for the interior of the bus. Teasing, verbal harassment or other verbal annoyance will not be tolerated. Any fighting, pushing, punching or other inappropriate physical contact will not be tolerated. Students can play portable electronic games on the bus under their own risk and responsibility. (FRIDAYS ONLY) Students can write or draw while on the bus, however they must not put pens in their mouth. Drawing papers must be on their laps, not the seat in front of them. No drawing on the bus or on other property. Parents are required to inform to the Eagle Bus Service office when students will not take the afternoon bus. School rules will be enforced at all times while on the bus. No student may ride the bus unless arrangements have been made at the Eagle Bus Service office. Students are responsible to be waiting for the bus at the designated location in the morning. The bus driver will not wait for tardy students. Students will also be responsible for getting off at the right location in the afternoon. If parent is not at stop, child will be returned to Academy of Martial Arts for afterschool care. Additional fees will apply. Students must report directly to the bus upon dismissal from school. Students must be seated directly after boarding the bus and remain seated until getting off the bus. Students must not make loud noises, such as yelling or screaming, or create distractions in any way. Students must not throw objects inside the bus or out of the windows. Students must not extend any objects, head, hands or feet out of the windows of the bus. Students will be given a warning card each time they violate the school bus rules. Any fighting or physically violent incidents will result in immediate suspension from the bus. Students will be suspended from the bus as follows; 1. At the first card, not suspended. 2. At the second card, suspended from the bus for one day. 3. At the third card, suspended from the bus for two days. 4. At the fourth card, suspended from the bus for three days. 1. Voluntary Participation/Permission. I am the parent of legal guardian (“Responsible Party”) of the minor participant named above. I have voluntarily given my Minor Child (“Rider”) permission to be transported by Eagle Bus Service, Inc. to and from school and during any and all school days, events and/or trips in which the Minor Child participates during the school season, without any limitation. I accept full responsibility for any injury or accident to my Minor Child, as the case may be, that may occur as a result of my Minor Child’s transportation by Eagle Bus Services, Inc. for the various events and or/trips as described above. 2. Assumption of the Risk. While Eagle Bus Services, Inc. takes safety measures to minimize the risk of injury to persons who the company provides school bus transportation services, Eagle Bus Services cannot guarantee that the participants, equipment, roads, all drivers, and other factors will be free from hazards, accidents, and/or injuries. I am voluntarily permitting My Child to be provided school bus services by Eagle Bus Services, Inc. with knowledge of such risk. I grant this permission with full knowledge that I accept full responsibility for any injury or accidents that may occur. 3. Injury to Persons or Property. The Responsible Party hereby voluntarily releases, discharges, waives and relinquishes any and all actions or causes of action for personal injury, property damage or wrongful death occurring to the Minor Child arising as a result of engaging or receiving transportation in said activity or any activities incidental thereto wherever or however the same may occur and continue, and the undersigned does for him/herself, his/her heirs, executors, administrators and assigns hereby release, waive, discharge and relinquish any action or causes of action, aforesaid, which may hereafter arise for him/herself and for his/her estate, and agrees that under no circumstances will he/she or his/her heirs, executors, administrators and assigns prosecute, present any claim for personal injury, property damage or wrongful death against Eagle Bus Service or any of its officers, agents or employees for any of said causes of action, whether the same shall arise by the negligence of any of said persons, or otherwise. The Responsible Party specifically intends to exempt and relieve Eagle Bus Service from liability for personal injury, property damage or wrongful death caused by negligence. 4. Indemnify, Defend, and Hold Harmless. The Responsible Party, for him/herself, his/her heirs, executors, administrators or assigns, agrees that in the event any claim for personal injury, property damage or wrongful death shall be prosecuted against Eagle Bus Service, he/she shall indemnify and save harmless the same from any and all claims or causes of action by whomever or wherever made or presented for personal injuries, property damage or wrongful death.
